Join Chartwell Midwest as a Home Health RN Case Manager – $5K Onboarding Bonus in Middleton, WI, US. Join our Home Health Agency team as an RN Case Manager! Key Responsibilities
The RN Case Manager coordinates and directs patient care by performing comprehensive assessments and developing individualized care plans. They work collaboratively with an interdisciplinary team to ensure high-quality home health services and maintain accurate clinical documentation.
Requirements
Candidates must hold a valid Wisconsin nursing license, a driver's license, and at least one year of recent nursing experience. An associate degree in nursing is required, while a bachelor's degree is preferred.
