Key Responsibilities
The RN Navigator will facilitate care coordination within the Clinically Integrated Network to ensure seamless transitions for patients in post-acute settings. They will work closely with the clinical team to provide person-centered care and support the mission of the organization.
Requirements
The candidate must be a Registered Nurse with experience in clinical settings. Strong communication skills and a commitment to compassionate, patient-centered care are required.
