Key Responsibilities
The Hospice RN Case Manager coordinates the total plan of care for patients, managing their needs from admission through discharge. They provide specialized nursing care, perform clinical assessments, and collaborate with an interdisciplinary team to support patients and families facing terminal illness.
Requirements
Candidates must be graduates of an accredited nursing program and hold a current Registered Nurse (RN) license in the state of practice. At least one year of hospice experience within the last 24 months and a valid driver's license with reliable transportation are required.
