Key Responsibilities
The Physical Therapist performs comprehensive patient assessments, develops individualized plans of care, and implements physical therapy modalities in a home health setting. They also collaborate with the interdisciplinary team, provide patient and caregiver education, and maintain accurate clinical documentation via Electronic Health Records.
Requirements
Candidates must hold a valid physical therapy license and have graduated from an accredited physical therapy program. Prior home health experience is preferred, along with a valid driver's license, reliable transportation, and current CPR certification.
