Job Details: Job Location: ACCESSPOINT RI - CRANSTON, RI, Position Type: Full Time, Salary Range: $40.00 - $42.00Hourly, Job Shift: Day, Position Summary The Nurse Case Manager provides clinical oversight, care coordination, and nursing support for individuals receiving services within AccessPoint RI’s residential programs. The Nurse Case Manager works collaboratively with individuals, families, residential staff, healthcare providers, pharmacies, and members of the interdisciplinary team to ensure that each individual’s healthcare needs are appropriately assessed, coordinated, documented, and addressed. This position serves as an important clinical resource to residential managers and Direct Support Professionals (DSPs), promoting safe medication practices, continuity of care, preventative healthcare, and compliance with applicable nursing and regulatory requirements. Essential Duties and Responsibilities Nursing Case Management and Clinical Oversight Provide nursing case management and clinical oversight for an assigned caseload of individuals living in residential settings. Review each individual’s medical history, diagnoses, medications, treatment plans, allergies, healthcare needs, and identified risks. Develop, maintain, and update nursing assessments, healthcare plans, protocols, and other required clinical documentation. Monitor individuals with acute and chronic health conditions and communicate significant changes to appropriate healthcare providers and members of the interdisciplinary team. Coordinate follow-up care after emergency department visits, hospitalizations, procedures, or changes in medical status. Participate in discharge planning and transitions of care as appropriate. Promote preventative healthcare and ensure recommended screenings, immunizations, laboratory testing, and follow-up appointments are addressed. Healthcare Coordination Coordinate medical appointments, specialist services, laboratory work, diagnostic testing, and other healthcare services. Review physician orders and ensure new or revised orders are appropriately communicated and implemented. Communicate with primary care providers, specialists, pharmacies, hospitals, families, guardians, and other healthcare partners. Review consultation reports and medical recommendations and ensure appropriate follow-up. Assist residential teams in understanding and implementing healthcare recommendations and restrictions. Advocate for individuals to ensure effective communication and continuity of healthcare services. Medication Management Provide clinical oversight of medication administration practices within assigned residential programs. Review medication orders and Medication Administration Records (MARs) for accuracy and consistency. Coordinate with pharmacies and healthcare providers to resolve medication discrepancies or concerns. Monitor medication changes, potential adverse effects, and effectiveness and communicate concerns to prescribing providers. Conduct medication and clinical record reviews as assigned. Support medication administration training, competency assessment, and ongoing education for qualified staff in accordance with organizational and regulatory requirements. Identify medication errors or variances and participate in follow-up, education, and corrective actions as appropriate. Residential Staff Support and Training Serve as a nursing resource to Residential Directors, managers, DSPs, and other program staff. Provide education regarding individuals’ diagnoses, healthcare protocols, medications, signs and sym

  • ptoms requiring intervention, and changes in medical status. Develop individualized health-related training when necessary. Reinforce infection-control practices, universal precautions, and other health and safety standards. Support orientation and ongoing healthcare training for residential employees. Collaborate with the Training and Quality team on nursing-related education and competency initiatives. Clinical Response and Risk Management Respond to clinical concerns and provide nursing guidance regarding changes in an individual's health status. Assist staff in determining appropriate clinical follow-up, including contact with healthcare providers, urgent care, emergency services, or other resources. Review incidents involving illness, injury, medication errors, hospitalization, or other healthcare concerns. Identify trends or areas of clinical risk and recommend preventative or corrective actions. Participate in root-cause reviews, quality-improvement activities, and clinical investigations as assigned. Follow mandatory reporting requirements and organizational procedures regarding suspected abuse, neglect, mistreatment, or other reportable events. Interdisciplinary Collaboration Participate in interdisciplinary team meetings, individual service planning meetings, clinical reviews, and other meetings as appropriate. Collaborate with Residential Services, Clinical Services, Behavioral Services, Nutrition, therapy providers, and other departments to support coordinated care. Provide nursing input regarding individual support plans and healthcare goals. Develop positive working relationships with individuals, families, guardians, healthcare providers, and community partners. Documentation and Compliance Maintain complete, accurate, and timely nursing documentation. Ensure clinical records contain required healthcare information, physician orders, assessments, treatment documentation, and follow-up information. Participate in internal audits and regulatory reviews. Assist residential programs in maintaining readiness for licensing, quality, and regulatory reviews. Maintain confidentiality and comply with HIPAA and organizational privacy requirements. Follow applicable professional nursing standards, organizational policies, and state and federal requirements.

Qualifications

Qualifications Required Current, unrestricted Registered Nurse (RN) license in the State of Rhode Island. Graduate of an accredited nursing program. Current CPR certification. Strong clinical assessment, nursing judgment, communication, and organizational skills. Ability to independently manage multiple individuals with varying healthcare needs. Ability to communicate effectively with individuals with intellectual and developmental disabilities, families, staff, and healthcare professionals. Strong documentation and electronic recordkeeping skills. Valid driver's license and ability to travel between residential locations and healthcare appointments as required. Preferred Bachelor's degree in Nursing (BSN). Two or more years of nursing experience. Experience working with individuals with intellectual and developmental disabilities (IDD), behavioral health needs, or complex medical conditions. Physical and Work Requirements The position requires regular travel among residential and program locations. The employee must be able to respond to clinical needs within community-based environments and perform the physical functions necessary to complete nursing assessments and provide clinical assistance when needed. Some flexibility outside of regularly scheduled hours may be required based on individual healthcare needs, urgent clinical matters, or program needs.

Requirements

A current, unrestricted Rhode Island RN license, graduation from an accredited nursing program, current CPR certification, and a valid driver's license are required. Strong clinical assessment, communication, organization, and documentation skills are needed; a BSN, two or more years of nursing experience, and experience with IDD or residential services are preferred.
