Key Responsibilities
Develop and implement individualized care plans while providing one-on-one support and assessments for hospice patients and their families. Collaborate with interdisciplinary teams to monitor patient conditions and ensure the delivery of high-quality end-of-life care.
Requirements
Must possess a valid Registered Nurse license in the state of employment and a current CPR certification. A valid driver's license and reliable transportation are required for patient visits.
