What You'll Do

As a Home Health PT, you will deliver personalized therapy while helping patients safely regain function and independence:

* Evaluate mobility, strength, balance, pain, and functional limitations in the home environment

* Develop and update individualized therapy plans aligned with physician orders and patient goals

* Provide therapeutic exercise, hands-on interventions, gait training, and mobility instruction

* Recommend and train patients on appropriate assistive devices and safety strategies

* Educate patients and caregivers on exercises, fall prevention, and safe techniques

* Track progress, adjust interventions, and maintain timely, accurate documentation

* Collaborate with nursing, occupational therapy, speech therapy, and other care team members

What You Bring

* Graduate of an accredited physical therapy program with an active Arizona PT license

* Minimum of 1 year of physical therapy experience

* Home health experience preferred 

* Current CPR certification

* Valid driver's license, auto insurance, and reliable transportation

* Strong clinical judgment, communication, organization, and documentation skills
