AdvantageCare Rehabilitation is looking for a PRN Physical Therapist (PT) to join our rehabilitation team at Grand Rapids Care Center in Grand Rapids, Ohio.

What Will You Do as a Physical Therapist?

As a Physical Therapist at Grand Rapids Care Center, you'll evaluate and treat patients with a variety of mobility and functional needs.

Your Role May Include

• Performing comprehensive Physical Therapy evaluations
• Developing individualized plans of care based on patient needs and goals
• Addressing strength, balance, gait, transfers, endurance and functional mobility
• Providing therapeutic exercise, neuromuscular re-education and functional mobility training
• Assessing the need for assistive devices and providing appropriate training
• Developing fall-prevention and safety strategies
• Monitoring patient progress and modifying treatment plans when appropriate
• Educating patients, families and caregivers
• Collaborating with PTAs and the interdisciplinary care team
• Completing accurate and timely clinical documentation
• Helping patients work toward greater safety, mobility and independence

What We're Looking For

• Graduate of an accredited Physical Therapy program
• Current or eligible Ohio Physical Therapist license
• Strong clinical judgment and communication skills
• Ability to work collaboratively with PTAs and the interdisciplinary team
• Skilled nursing, geriatric or rehabilitation experience is a plus
• A patient-first approach to care
