Key Responsibilities
The RN Case Manager will assess patient needs, create personalized hospice care plans, and provide direct nursing care including medication administration and symptom management. They will also collaborate with an interdisciplinary team and provide education and support to patients and their families.
Requirements
Candidates must be a graduate of an accredited nursing school with a current nursing license and have at least one year of nursing experience. A valid driver's license and insured vehicle are required for travel within the coverage area.
