Key Responsibilities
The Hospice Registered Nurse Case Manager is responsible for coordinating hospice care and leading the interdisciplinary team to meet patients' physical, emotional, and spiritual needs. They conduct nursing assessments, manage symptoms, and provide education and support to patients and their families throughout the end-of-life process.
Requirements
Candidates must be graduates of an accredited nursing school and hold a valid Colorado RN license. At least one year of experience in med-surg, emergency, or critical care nursing is required.
