Case Management - Registered Nurse - Telehealth ( MT or PST)

Key Responsibilities

The Case Manager RN is responsible for assessing, planning, and coordinating case management activities to evaluate and address the medical, behavioral, and social needs of members. They will use clinical tools and motivational interviewing to engage members, reduce risk factors, and improve overall health outcomes.

Requirements

Candidates must be a Registered Nurse with an unrestricted multi-state/compact license and reside in the Continental United States within MT or PST zones. The role requires at least 3 years of clinical nursing experience and 1 year of medical case management, care coordination, or discharge planning experience.
