Hospice Registered Nurse Case Manager

Key Responsibilities

The RN Case Manager is responsible for coordinating hospice care and managing the physical, emotional, and spiritual needs of patients. They conduct nursing assessments, implement care plans, and provide education to patients and families regarding end-of-life processes.

Requirements

Candidates must be graduates of an accredited nursing school with a valid Texas RN license and at least one year of experience in med-surg, emergency, or critical care. Strong clinical assessment, communication, and time management skills are required.
