This position offers weekends off, day shifts only. Work with Altus Hospice in Marble Falls, Texas.
The RN Case Manager will provide hands-on hospice nursing care and coordinate services for patients within the Marble Falls community. They will also collaborate with the clinical team to assess patient eligibility and provide compassionate support to families throughout the hospice journey.
Candidates must hold a current, unencumbered Registered Nurse license and possess at least one year of experience in a medical-surgical or acute care setting. Proficiency in EMR systems, a valid driver's license, and current CPR certification are also required.
