Key Responsibilities

The RN Case Manager conducts comprehensive telephonic assessments and develops person-centered care plans for members with complex medical and social needs. They coordinate care across various health systems and facilitate safe transitions between care settings to ensure members remain safely in their preferred environment.

Requirements

Candidates must hold an active, unrestricted RN license and have at least two years of professional nursing experience. Proficiency in electronic health records and experience with Medicaid or managed care programs are required.
