This position offers no call requirements, weekends off. Work with Crescent Hospice in North Charleston, South Carolina.
The RN Case Manager will deliver hands-on hospice nursing care and coordinate services for patients and families within the community. They will also collaborate with the Clinical Director to assess patient eligibility and participate in interdisciplinary care planning.
Candidates must hold a current unencumbered Registered Nurse license and have at least one year of experience in a medical-surgical or acute care setting. Proficiency in EMR systems and a valid driver's license with reliable transportation are also required.
