Physical Therapist - (PT) PRN

PRN Physical Therapist

Assess, plan, organize, and provide rehabilitative services that improve mobility, relieve pain, increase strength, and prevent or correct disabling conditions resulting from illness or injury.

As a PRN Physical Therapist, you will

* Perform and document initial evaluations, analyze clinical findings, identify patient needs, and establish a diagnosis before intervention.

* Develop and implement individualized treatment plans to maintain, improve, or restore physical function, alleviate pain, and prevent further dysfunction.

* Evaluate patients’ responses to treatment and adjust plans of care to achieve maximum benefit.

* Apply infection-control standards and universal precautions while providing patient care.

* Ensure continuity and quality of care through accurate, timely documentation.

* Communicate effectively with patients, families, team members, physicians, and other healthcare professionals.

* Promote and uphold the agency’s philosophy, policies, and procedures.

* Complete all required documentation within company-established guidelines.

* Accept patient assignments based on availability and organizational needs.

Requirements

* Successful completion of an accredited physical therapy program.

* Current, valid Physical Therapist license in the state of practice.

* Minimum of one year of home health experience preferred.

* Current CPR certification.

* Valid driver’s license and automobile insurance.

* Homecare Homebase experience preferred.

* Ability to work a flexible PRN schedule based on patient-care needs.

Physical Demands

* Ability to stand, walk, sit, climb, crawl, squat, and kneel for extended periods.

* Ability to lift, carry, pull, or push up to 100 pounds.

* Ability to travel to patients’ homes within the assigned service area.
