Key Responsibilities

The provider will deliver comprehensive pediatric care including diagnosis, treatment, and preventive services within a community health center. Responsibilities also include maintaining electronic health records, coordinating care teams, and participating in patient education and staff training.

Requirements

Candidates must hold a Doctor of Osteopathy or Doctor of Medicine degree and have completed a primary care residency with board certification. A minimum of two years of outpatient primary care experience is preferred, along with current Illinois licensure.
