Key Responsibilities
The Registered Nurse Case Manager conducts comprehensive patient assessments and develops individualized care plans in collaboration with multidisciplinary teams. They travel to patient residences to provide hospice care, coordinate with families, and educate patients on treatment and medication management.
Requirements
Candidates must be a licensed Registered Nurse with BLS certification and at least one year of clinical experience. A valid driver's license, auto insurance, and reliable transportation are required for travel to patient homes and facilities.
