Job Description
About GoldenScript
Every year, millions of patients are hospitalized because of medication problems, and billions of dollars in taxpayer money are wasted on drugs that deliver little therapeutic value. The reason is structural: no single entity in healthcare looks across a patient's entire medication list to make sure they're on the right drug, at the right dose, at the right time, for the right price.
That's the gap GoldenScript fills. We are a pharmacy services platform that traverses the complex maze of clinical, insurance, and financial data behind every prescription, combining it into a comprehensive view of each patient's optimal medication list. From there, our clinical team executes the last mile: coordinating prescribers, pharmacies, and payers to actually move each patient onto the right regimen.
The Role
GoldenScript analyzes lab results to understand whether a patient's medications are actually working and how well they're tolerating them. None of that exists until a clinician sits down with the patient, asks how they're doing, and orders the testing.
You're an NP on our Patient Safety team, performing telehealth visits with patients to screen for side effects and order the lab testing that helps us understand their medications better.
The screening is where something live can surface — a patient who's been dizzy since their dose changed — and you're the first clinician to hear it.
A full schedule of short visits is the shape of this job.
Visits run over video, and you'll work across both of our programs: scheduled visits where you join from a home health aide's tablet once they're in the home with the patient, and live handoffs where a patient is brought into your waiting room and you pick them up there.
The clinical protocol is set before you join the call, so this is high-volume execution rather than complex clinical decision-making. There is no panel to carry and no follow-up schedule of your own, and you measure the day in patients seen rather than cases worked.
The pace is uneven: stretches of back-to-back visits, and stretches of waiting for the next patient to appear. For the right NP, that's the appeal: nothing about a patient's medications gets safer until a clinician asks the right questions and orders the right test, and you're the clinician who does.
What You'll Do
- Run a full day of telehealth visits — scheduled home visits at roughly sixteen a day, or live handoffs at a higher volume — and pick up unscheduled visits when the day opens up
- Ask a standardized set of screening questions about how each patient is feeling and tolerating their medications, and capture any conditions they report that aren't already in the chart
- Obtain the patient's consent, order whatever testing their situation calls for, and make sure they know what happens next — whether the aide collects the sample with you on the call or the kit goes to the patient's home
- Document every visit in our EHR
- Escalate to our clinical team when something falls outside the scope of a short visit: a patient reporting new or worsening symptoms, a possible adverse reaction, or anything that calls for a clinical decision rather than documentation. A patient with a lot of questions isn't an escalation. That part is the job
We're Looking for Someone Who Is
- A clear, warm communicator who can put a patient at ease in the first thirty seconds, including people who are older, worried, or hard of hearing
- Efficient without sounding rushed. You can cover everything a visit needs inside the time you have and still leave the patient feeling heard
- At home in a startup: our tools, scripts, and workflows change as we learn, and you'd rather help shape them than wait for a finished playbook
- Disciplined about documentation, because the visit isn't done until it's written up
- Anchored by the belief that what you're doing is genuinely good for the patient — that conviction is what keeps the last visit of the day sounding like the first
Required Qualifications
- Active, unrestricted New Jersey license as a Nurse Practitioner, with current national board certification
- 2+ years of practice as an NP; primary care, geriatrics, home health, telehealth, and care management all count
- Strong working knowledge of common drug classes and the side effects patients actually notice and report — enough to know when a screening answer matters and when it doesn’t
- Comfortable carrying a full schedule of short, protocol-driven visits — sustained visit volume is a core daily activity in this role
- Exceptional attention to detail and documentation discipline; comfortable working inside a structured documentation system and across multiple EHRs
Nice to Have
- Medication management or Medicare experience
- Prior telehealth experience at volume
- Startup or fast-moving clinical environment experience
