What You'll Do
- Complete in-home physical therapy evaluations
- Develop individualized treatment plans
- Improve mobility, strength, balance, and function
- Educate patients and caregivers
- Coordinate care with physicians and clinicians
- Maintain clinical documentation

Key Responsibilities
The Physical Therapist will conduct in-home evaluations and develop individualized treatment plans to improve patient mobility and function. They are also responsible for educating caregivers and coordinating care with physicians and clinicians.
Requirements
Candidates must possess an active Florida PT License and strong assessment and treatment skills. Previous experience in home health is preferred, along with the ability to work independently.
