Key Responsibilities

The Nurse Practitioner provides comprehensive, integrated care to complex CKD/ESKD patients through both in-person visits and telehealth. They function autonomously to coordinate care, manage comorbidities, and reduce hospitalizations in collaboration with the healthcare team.

Requirements

Candidates must have at least two years of experience as a Nurse Practitioner and hold an unrestricted NP license in Connecticut, with the ability to obtain one in New York. Proficiency in clinical assessment, valid driver's license, and access to a home office with high-speed internet are required.
