Conduct comprehensive in-home health evaluations, including physical exams and medical history reviews, to bridge gaps in care. Build trusted relationships with patients and provide connections to appropriate healthcare services based on unique needs.
Requirements
Requires an active, unrestricted license and board certification as a Nurse Practitioner or Physician Assistant. Ability to travel locally within licensed states and conduct virtual visits is necessary.
