• Mileage Reimbursement: $0.60/mile for distance covered between patient home appointments
• Unit: Home Setting
• Shifts available: Days
• Shift timings: 8 Hours; Days, 8.00a – 5.00p

Schedule: 40 hours / week guaranteed, 5 shifts per week, 5 X 8-hr shifts, Monday to Friday, with every 4th weekend (or) Thursday to Monday (or) Tuesday to Saturday

Position Requirements

• Registered Nurse: 2 years - Required
• Recent/Current Home Health RN experience, 2 years – Required
• OASIS experience - Required
• HCHB EMR experience - Preferred
• Must have valid Driver’s License and Personal Auto Insurance

Education, License and Certification Requirements

• ASN or an equivalent – Required
• BSN or an equivalent – Highly preferred
• RN License for State/s applying or Compact - Required
• BLS Certification from AHA or American Red Cross - Required

Job Description

Primarily provide RN Case Management for Home Health patients

RN Case Managers screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with the interdisciplinary team members and the patient.

Develops a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission. Periodically reviews the patient care plan and recertifies the patient care plan as appropriate.

Coordinate the patient's care plan through interdisciplinary communication and case conferences, as appropriate. Advise other disciplines of discharges in a timely manner.

Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals.

• Communicate significant changes to the patient's physician and colleagues.
• Document evaluations, treatment goals, and objectives.
• Teach the patient and caregiver self-care techniques as appropriate.

Provide medication, diet, and other instructions as ordered by the physician and utilizes opportunities for health counseling with patients and caregivers.
