- $2720 total weekly gross - $68/hour ($31/hour W-2 taxable, $37/hour non taxable stipend)
Requirements: 3+ years Acute Care Case Management experience
Must reside outside a 60 mile radius
Key Responsibilities
The RN Case Manager is responsible for assessing discharge needs and managing social determinants of health for acute medical and surgical patients. They will also handle discharge planning and referrals for a caseload of approximately 20 patients per day.
Requirements
Candidates must have at least 3 years of acute care case management experience. Additionally, the applicant must reside outside a 60-mile radius of the facility.
