Hospice Physician – Provide Expert, Compassionate Care When It Matters Most
Are you a physician who values compassionate, patient-centered care and meaningful collaboration? Home Healthcare, Hospice and Community Services is seeking a per diem Hospice Physician to support the medical needs of hospice patients and provide clinical guidance to our interdisciplinary care team. Reporting to the Hospice Medical Director, you will work closely with primary care physicians, hospice staff, patients, and families to help ensure thoughtful medical oversight, effective pain and symptom management, and care that supports comfort and dignity throughout the hospice journey. This position is primarily remote, generally working 15–24 hours per week, with participation in a weekend rotation.

Key Responsibilities
Collaborate with the interdisciplinary care team to address patients' medical needs and provide oversight of the medical plan of care. You will work closely with physicians, hospice staff, patients, and families to help ensure thoughtful medical oversight, effective pain and symptom management, and care that supports comfort and dignity throughout the hospice journey.
Support effective pain and symptom management for hospice patients.
Serve as a clinical resource to hospice staff, patients, families, attending physicians, and other community providers.
Assist primary care physicians and hospitalists with medical history, physical information, and medical management as appropriate.
Provide guidance related to medicine, disease processes, pharmacology, pain and symptom control, and the psychological aspects of loss and grief.
Determine and document hospice medical eligibility, including completion and signature of the Certification of Terminal Illness (CTI).
Complete required initial, 90-day, and subsequent face-to-face 60-day recertifications.
Participate in Hospice interdisciplinary team meetings when the Hospice Medical Director is unavailable.
Provide clinical guidance in resolving conflicts and addressing ethical concerns.
Participate in Continuous Quality Improvement activities and support hospice quality initiatives.
Maintain compliance with applicable hospice regulations, organizational policies, the Corporate Compliance Plan, and Standards of Conduct.
Requirements
Required
Physician duly licensed under the provisions of the State of New Hampshire.
Comprehensive knowledge and skill related to: hospice benefit eligibility guidelines, hospice regulations, medicine, disease processes, pharmacology, pain and symptom control, and psychology of loss and grief.
Visual and hearing acuity sufficient to perform the responsibilities of the position.
Effective communication skills.
Preferred
Board Certification in Hospice and Palliative Care, or willingness to become board certified when eligibility criteria are met.
Hospice Medicare eligible to order and certify
