What You'll Do

As a Home Health Physical Therapist, you will

* Perform comprehensive patient evaluations and establish individualized treatment plans.

* Develop and implement therapeutic interventions to improve mobility, strength, balance, and functional independence.

* Evaluate patient progress and modify treatment plans to achieve optimal outcomes.

* Educate patients and caregivers on exercises, safety, and disease management.

* Maintain accurate, timely documentation in accordance with company standards.

* Collaborate with physicians, nurses, and the interdisciplinary care team.

* Follow infection control and universal precaution guidelines.

* Deliver compassionate, high-quality care while promoting Choice Health At Home's mission and values.

Qualifications

* Graduate of an accredited Physical Therapy program.

* Current Physical Therapist (PT) license.

* Minimum of one (1) year of home health experience preferred.

* Current CPR certification.

* Valid driver's license and reliable transportation with current auto insurance.

* HomeCare HomeBase experience preferred.

Physical Requirements

* Ability to stand, walk, bend, kneel, squat, and sit for extended periods.

* Ability to safely lift, carry, push, or pull up to 100 pounds with appropriate assistance when necessary.
