Key Responsibilities
The physician will deliver comprehensive outpatient care to adult and senior patients, maintaining accurate medical records and diagnosing/managing moderately to complex medical conditions. Responsibilities also include coordinating referrals, participating in quality improvement initiatives, and working closely with interdisciplinary teams to ensure holistic care.
Requirements
Candidates must possess an MD or DO from an accredited medical school with an active, unrestricted medical license and Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine. A minimum of 2 years' experience in value-based care or managing high-acuity geriatric patients is required.
