Job Description
Responsible for collaborating with healthcare providers and physicians for problem solving, prioritizing and facilitating patient care objectives, the coordination and development of staff education, and discharge patient education and follow-up for the area assigned.

Key Responsibilities
The RN Case Manager collaborates with healthcare providers to prioritize and facilitate patient care objectives. They are also responsible for coordinating staff education and managing patient discharge planning and follow-up.
Requirements
Candidates must hold a current California RN license and have at least two years of recent clinical experience in an acute care hospital. Additional requirements include BLS certification from the American Heart Association and obtaining an LA City Fire Card within six months of hire.
