Key Responsibilities
Conduct comprehensive patient assessments and develop individualized plans of care while providing skilled nursing in home and facility settings. Coordinate effectively with physicians, caregivers, and the interdisciplinary team to ensure high-quality end-of-life support.
Requirements
Requires an active Registered Nurse (RN) license, a valid driver's license, and the ability to pass background and motor vehicle checks. Previous hospice or home health experience is preferred, along with proficiency in clinical documentation and care coordination.
