The physician will provide direct virtual care to high-risk patients recently discharged from acute settings to prevent readmissions. Additionally, the role involves clinical supervision of interdisciplinary teams, leading rounds, and driving quality improvement through AI-enabled tools.
Requirements
Candidates must hold an MD or DO degree with board certification in Internal Medicine, Family Medicine, or a related specialty. A minimum of 3 years of post-residency clinical experience and experience in transitions of care or inpatient settings is required.
