Language Skills - Ability to read, analyze, and interpret the most complex documents. Ability to respond effectively to the most sensitive inquiries or complaints. Mathematical Skills - Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. Reasoning Ability - Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. Computer Skills - To perform this job successfully, an individual should have knowledge of Athena Database software; Microsoft Spreadsheet software and Microsoft Word Processing software; including Excel. Key Responsibilities

The Advanced Practice Provider is responsible for managing total patient care, including assessments, care planning, and acute interventions in a home-based setting. They must also ensure accurate documentation in the electronic medical record and coordinate care with the medical team and community resources.

Requirements

Candidates must hold a master's degree in nursing and possess an Advanced Registered Nurse Practitioner license. Strong clinical skills, proficiency with electronic medical records, and the ability to perform in-home patient visits are required.
