Key Responsibilities
The RN Case Manager is responsible for conducting comprehensive patient assessments, developing and revising individualized care plans, and coordinating care for an assigned caseload. They must also maintain clinical documentation, communicate patient status to physicians, and participate in on-call duties.
Requirements
Candidates must hold a valid RN license in the State of Idaho and possess a valid driver's license. Previous experience in Home Health and Hospice settings is preferred.
