Reports to: Clinical Team Manager Hours: Core work hours are 8am to 4:30pm Monday – Friday. Hours and days may vary depending on patient, organizational needs or if working scheduled weekend or holiday rotation.
This position offers day shifts only. Work with Hospice Alliance Inc in Pleasant Prairie, Wisconsin.
The RN Case Manager plans and delivers nursing care to terminally ill patients across various residential settings using the nursing process. They coordinate interdisciplinary care, document patient progress in electronic medical records, and participate in weekend and holiday rotations.
Candidates must be graduates of an accredited nursing school with a valid Wisconsin RN license and current Basic Life Support certification. At least one year of recent professional nursing experience is recommended, with preference given to those with hospice or palliative care backgrounds.
