Key Responsibilities
The Advanced Practitioner will deliver personalized, compassionate medical care primarily to individuals with CKD and/or ESRD/ESKD, focusing on improving quality of life and slowing disease progression. Responsibilities include conducting comprehensive annual wellness exams both virtually and in the home, coordinating care, and documenting patient information accurately.
Requirements
Candidates must possess an active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license, be board certified, and hold a current and unrestricted DEA certificate. A minimum of two years of direct patient care experience is required, along with access to transportation and proficiency with medical equipment and computer-based data management.
