Job Description

  • Job Title: Primary Care Physician (MD/DO)
  • Location: Rochester, NY (West Ridge Rd), clinic-based, in-person
  • Reports To: Senior Medical Director

About TSH

Town Square Health is reimagining what healthcare can be. We’re building a first-of-its-kind value-based care model with a singular mission: to set the gold standard for how Americans experience healthcare. Our primary care locations serve Medicare-eligible patients, and we’re growing fast, with expansion into multiple markets on the horizon.

Our model was built by Dr. David Buchanan, prior Oak Street Health executive and practicing internal medicine physician with 25 years of experience in value-based care serving Medicare and Medicaid patients.

Our breakthrough care model for Medicare patients combines relationship-driven primary care and specialty care into simultaneous visits.

Each patient is supported by a dedicated Navigator, a partner who builds long-term trust, eases caregiver burden, and leverages AI-enabled workflows to improve chronic disease outcomes.

Who We Are

We are a team of people who care deeply: about our patients, about each other, and about getting this right. What we’re building hasn’t been done before in healthcare, and that’s exactly the point. We ask hard questions, challenge assumptions, and aren’t afraid to try things that don’t come with a playbook.

We hold ourselves to a high standard, check our egos at the door, and show up for each other every day. If you want to do meaningful work alongside people who are genuinely invested in building something the industry hasn’t seen, you’ll fit right in here.

The Opportunity

This is not a traditional Primary Care Physician role.

Town Square Health is hiring a Primary Care Physician (MD/DO) for our new West Ridge Road location in Rochester, NY. You’ll deliver high-quality primary care in a Medicare-focused, value-based care environment, with the time, team support, and resources to practice comprehensive medicine: patient panels smaller than the national average, 20 to 40 minute visit lengths based on patient needs, and a team-based care model with two Navigators paired to each provider.

Our Navigators are an expanded Medical Assistant role. Alongside the clinical support you’d expect from an MA, each Navigator serves as a dedicated partner to their patients, building long-term relationships and supporting chronic disease management between visits.

You’ll join at an early stage with a chance to help build a care model from the ground up. That means real ownership, real influence, and the opportunity to shape how clinical excellence comes to life at TSH.

You’ll Have the Opportunity To

  • Deliver excellent clinical care with the time and team support to practice comprehensive medicine
  • Coordinate across the care team, partnering closely with your Navigators and specialty colleagues
  • Help shape a new care model designed to improve outcomes and patient experience

What You’ll Do

As a Primary Care Physician at TSH, you’ll own the care of your patient panel in a clinic built around relationships, not volume. You’ll lead a team-based care model, use AI-enabled tools like AI scribes, and help build the clinical workflows our future clinics will follow.
