Job Duties
Assess, plan, develop and provide the patient or responsible caregiver a home health plan of care for occupational therapy. identifies patient needs for other disciplines and services.
Coordinate and evaluate the plan of care with all other disciplines, organizations and physicians for an assigned caseload.
Communicates significant changes in patient status to the physician, rehab coordinator and the appropriate agency personnel when indicated.
Demonstrates effective communication skills, keeping lines of communication open between Lead O.T. Supervisor, Team Managers and all staff involved in care of assigned patients.
Minimum Requirements
- 2 years of experience as an occupational therapist
- Current registration to practice Occupational Therapy in New York State
Salary Range
USD $90,000.00 - USD $115,000.00 /Yr.

Key Responsibilities
The occupational therapist will assess, plan, and provide home health care for patients while coordinating with other disciplines and physicians. They are responsible for communicating significant changes in patient status to the appropriate agency personnel.
Requirements
Candidates must have at least 2 years of experience as an occupational therapist. A current New York State registration to practice Occupational Therapy is required.
