Key Responsibilities
The RN Case Manager will provide clinical care and coordinate services for patients in their homes. They are responsible for managing patient care plans and ensuring high-quality health outcomes.
Requirements
Candidates must be a licensed Registered Nurse. Previous experience in home health or a similar clinical setting is typically required for this role.
