Key Responsibilities
The Nurse Navigator coordinates individualized care for patients throughout the continuum of services to ensure timely access to healthcare. They serve as a key point of contact, patient advocate, and liaison within a multidisciplinary team to eliminate barriers to care.
Requirements
Candidates must hold an active Connecticut RN license and possess a minimum of 5 years of clinical nursing experience. A Bachelor of Science in Nursing is preferred for this role.
