Join Our Team as a Full-Time Physical Therapist (PT)

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our in-house team of outstanding professionals. In this role, you will provide person-centered care to help residents achieve positive outcomes and improve their mobility, strength, and overall quality of life.

Your Impact as a Physical Therapist (PT)

* Consult with patients to assess their physical condition and functional limitations

* Evaluate and interpret patient assessments and test results

* Develop individualized treatment plans using a variety of therapy techniques

* Create personalized rehabilitation programs tailored to each patient’s goals

* Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing

* Ensure all treatments are delivered in accordance with the established plan of care

* Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team

* Provide clinical support and education to patients, family members, and caregivers

* Collaborate with interdisciplinary team members to ensure high-quality patient care

* Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations

* Comply with all applicable rules, regulations, and procedures

* Attend required meetings as designated by the Director of Rehab

What Makes You a Great FIT

* Active and valid Texas Physical Therapist license

* In good standing with all regulatory agencies and licensing boards

* Skilled Nursing Facility (SNF) or long-term care experience preferred

* Current knowledge of physical therapy treatment practices

* Experience with Net Health Electronic Documentation System preferred

* Ability to manage patients with varying personalities and needs

* Strong communication, collaboration, and clinical decision-making skills
