Key Responsibilities

The RN Case Manager is responsible for the clinical management of a hospice patient caseload, including initial assessments and ongoing care evaluations. They coordinate with the interdisciplinary team to develop and implement patient-centered care plans that meet end-of-life goals.

Requirements

Candidates must hold a state-issued RN license and have at least two years of administrative experience, preferably in hospice or discharge planning. A bachelor's degree is preferred, along with proficiency in Microsoft Office and clinical documentation software.
