The Nurse Navigator coordinates comprehensive care for patients with serious illnesses across inpatient and outpatient settings. They develop individualized care plans and facilitate communication between interdisciplinary teams to ensure goal-concordant care.
Candidates must hold an Associate of Nursing degree and an active Registered Nurse license in Indiana or a compact state. A minimum of 3-5 years of clinical experience in complex care environments is preferred.
