- You will be responsible for seeing patients virtually and in person, via
scheduled appointments and walk ins. Position involves a wide range of responsibilities including but not limited to: crisis intervention, assessing for safety using evidence based screening, concrete services, assisting and following up with home care, psychosocial assessments, referrals to CBO's. Must work collaboratively with interdisciplinary team and across the network of ambulatory care clinics.
- This is a Monday - Friday 1199 position working 9:00 AM - 5:00 PM.

Key Responsibilities
The social worker will provide crisis intervention, psychosocial assessments, and safety screenings for patients in an outpatient internal medicine setting. They will work collaboratively with an interdisciplinary team to coordinate care, referrals, and home care services for patients.
Requirements
Candidates must possess a Master's Degree in Social Work (MSW) and a New York State Licensed Masters Social Worker (LMSW) license. Experience in a busy outpatient setting and working with interdisciplinary teams is preferred.
