Key Responsibilities

The Nurse Case Manager performs care management for members with complex and chronic needs by assessing, developing, and monitoring individualized care plans. They coordinate with medical groups, providers, and community resources to ensure appropriate service access and effective discharge planning.

Requirements

Requires a bachelor's degree in a health-related field and at least 3 years of clinical experience, or an equivalent combination of education and experience. A current, unrestricted RN license in the state of California is mandatory.
