The Telephonic Nurse Case Manager performs care management for members with complex and chronic needs by assessing, developing, and monitoring individualized care plans. They coordinate internal and external resources while interfacing with Medical Directors to optimize health outcomes across the care continuum.
Candidates must possess a Bachelor's degree in a health-related field and at least 3 years of clinical experience, or an equivalent combination. A current, unrestricted RN license is required, along with the ability to obtain multi-state licensure.
