Case Manager RN-Field

Key Responsibilities

The Case Manager RN is responsible for assessing, planning, and coordinating case management activities to evaluate and support the medical needs of members. They conduct both telephonic and face-to-face visits to develop proactive care plans that enhance member wellness and health outcomes.

Requirements

Candidates must be a Registered Nurse with a current, unrestricted West Virginia license and at least 5 years of clinical practice experience. The role requires the ability to travel 50% or more of the time within the assigned region and proficiency in Microsoft Office applications.
