RN Care Coordinator (Case Manager) BWH

Key Responsibilities

The RN Care Coordinator manages a caseload of patients to ensure clinical and financial outcomes by coordinating care across the continuum from admission to discharge. They facilitate interdisciplinary communication, execute discharge plans, and perform utilization management to ensure appropriate resource use and reimbursement.

Requirements

Candidates must be a graduate of an approved nursing school with current Massachusetts registration and a Bachelor of Science in Nursing for newly licensed nurses. The role requires at least 5 years of acute care experience and 1 year of inpatient acute case management experience.
