Case Manager (RN)

Key Responsibilities

The Case Manager coordinates skilled care for residents according to physician orders and facilitates communication between the interdisciplinary team, residents, and families. They are responsible for conducting pre-admission assessments, managing MDS documentation, and overseeing the quality assurance process.

Requirements

Candidates must be a Registered Nurse with a current license from the State Board of Nursing and have graduated from an accredited nursing school. Experience in a similar role with strong interpersonal and organizational skills is required.
