Key Responsibilities
The physician will manage a panel of up to 450 senior patients, providing comprehensive, value-based primary care for acute and chronic conditions. They are responsible for leading a dedicated care team, coordinating care across the continuum, and meeting clinical and quality performance goals.
Requirements
Candidates must hold an MD or DO degree in Internal Medicine, Family Medicine, or Geriatrics and possess or be able to obtain a Florida medical license. Board eligibility is required, along with a current DEA number and the ability to travel for initial training.
