Key Responsibilities
The physician will provide outpatient primary care to a senior patient population within a value-based care model. Responsibilities include managing complex chronic conditions and working with a dedicated care team to ensure continuity of care.
Requirements
Candidates must hold an MD or DO degree and be board certified or eligible in Family or Internal Medicine. A minimum of 2 years of outpatient experience and an active, unrestricted medical license are required.
