Work Schedule

This position offers weekends off, hybrid flexibility. Work with Vynca in Northern New Jersey, New Jersey.

Key Responsibilities

The Nurse Practitioner will provide evidence-based clinical care to complex and seriously ill patients through a combination of telemedicine and in-person visits. They are responsible for symptom management, goals of care discussions, and coordinating transitions of care to improve patient outcomes and reduce hospitalizations.

Requirements

Candidates must have at least 2 years of experience providing care to complex, seriously ill patients and hold an unrestricted New Jersey RN and NP license. A graduate degree from an accredited nurse practitioner program and relevant certifications such as ANP, AGNP, or FNP are required.
